* Referral is done through Chronic Condition Management (CCM) items linked to a patient’s registration in MyMedicare, to support continuity of care for people with chronic and complex conditions. CCM items replace what were previously called Care Plans / GP Management Plans.
How to Register
Registration is restricted to regular patients of the medical centre (at least two face-to-face visits for practices in other locations in the previous 24 months). Parents/guardians and children can be registered at the same practice if one of them is eligible and registered.
- Start the registration process in your Medicare Online Account or Express Plus Medicare Mobile app. Practice staff will then accept the registration in the MyMedicare system.
- This is the simplest and quickest way to register.
- Fill out a registration form and provide it to your practice. By signing the form, you are giving consent to participate in MyMedicare. Practice staff will then complete the registration in the MyMedicare system.
